Community Support

Intensive Community Rehabilitation Services

This new program offers individuals diagnosed with a serious and persistent mental illness a range of clinical and rehabilitation services to prevent hospitalizations, civil commitments, incarcerations and other adverse outcomes.  Services are designed to help participants develop their own support networks, enhancing their ability to live independently.

A collaboration of Zumbro Valley Mental Health Center, Olmsted County and Fillmore County, the program provides:

• Psychiatric services
• Education about medications and physical health
• Case management services
• Adult Rehabilitation Mental Health Services

Services are available to adults diagnosed with a serious and persistent mental illness who live in Fillmore and Olmsted counties.
